Established Patients Needing an Appointment
Breeze is our patient portal where patients can request appointments, communicate with our staff, update and fill out patient paperwork, and confirm your appointment. Our goal is to make the relationship with our practice as smooth and easy as possible!
New Patients Who Would like to Schedule an Appointment
If you are a new patient needing an appointment, please fill out the New Patient Appointment Request Form.
Once it is completed, we will be in touch with you via your preferred contact method.
If your preferred contact method was a text message, you will receive a text message from an (844) area code. There will be a link within that text message which will require you to access a secure portal to read and reply to the message. You will need to use the patient’s first and last name and date of birth provided on this form to access the secure portal. You will then be able to read and reply to the message there. If you are having issues getting to the secure portal, please call the office at (918) 747-4855.
We will also send you an email to activate your patient portal, Breeze, where you can complete patient registration by filling out the new patient paperwork and confirming your appointment. If you are unable to fill out the paperwork ahead of time, please arrive 15-20 before your scheduled appointment time to complete it in our office. Please be sure to bring your insurance card and driver’s license to your appointment.
